    We provide Case Management as well as specialized training and guidance for families and partners in aging. This includes: Assistance and training in managing end of life needs and requirements, Assistance and resources for updating Advanced Care Directives, and Referrals to qualified professionals (legal, etc.) to assist in completing and filing  guardianship, POAs, living wills, etc.

    Assess for in home safety as needed; assistance and/or referral to apply for Health and Human Services Community Attendant Services. This service is especially helpful for caregivers that aren’t local to the individual needing assistance. The program can help with many things- meal prep and shopping, bathing and hygiene, light household tasks.

    Sometimes we just need someone to talk to. My practice is open to new patients for psychotherapy- for adults 18+.

    Assessment and treatment for mental health concerns. The first contact will be a consultation to ensure therapy is appropriate and for you to get to know me; future appointments will include assessment of bio-psycho-social needs and discussion based on needs identified.

    This covers the initial consultation for services up to 1 hour

    Cost Per Service Hour = $100

    This is the cost per service hour

    Monthly Service Cost = $950

    This covers the cost of monthly services for up to 10 hours in 1 calendar month
